      目的通过观察谷氨酰胺(Gln)对慢性阻塞性肺疾病(COPD)患者外周血单个核细胞(PBMC)中p38丝裂原活化蛋白激酶(p38MAPK)活性、IL-8及IL-17表达的影响,探讨Gln在COPD患者治疗中的抗炎作用机制。方法将32例住院COPD急性加重期(AECOPD)患者设为AECOPD组,经治疗后转为稳定期(SCOPD)再设为SCOPD组。收集两组患者的PBMC,再平均分为加Gln干预的Gln组和不加Gln干预的空白对照组,另选择16例健康体检者为健康对照组。采用RT-PCR法检测各组PBMC中p38MAPK、IL-8及IL-17的表达水平。结果(1)AECOPD和SCOPD的空白对照组中p38MAPK、IL-8及IL-17的表达均高于健康对照组(均P<0.05),且急性期增高更加显著。(2)AECOPD的Gln组中p38MAPK、IL-8及IL-17的表达低于AECOPD空白对照组(均P<0.01);SCOPD的Gln组中p38MAPK、IL-8及IL-17的表达低于SCOPD空白对照组(均P<0.05)。结论Gln可抑制COPD患者炎症细胞中p38MAPK的活化并下调IL-8、IL-17的表达水平。

      Objective To investigate the effects of glutamine on the p38MAPK, IL-8 and IL-17 expression in peripheral blood mononuclear cells (PBMCs) of patients with chronic obstructive pulmonary disease (COPD). Methods Thirty-two patients with acute exacerbation of COPD(AECOPD ) and 32 patients with stable COPD(SCOPD) were enrolled in the study. Venous blood samples were collected and PBMCs were isolated. The patients were equally divided into the Gln intervention group and blank control group. Sixteen healthy subjects served as normal control group. The expression levels of p38MAPK, IL-8 and IL-17 in PBMCs were detected by real-time PCR. Results The expression of p38MAPK, IL-8 and IL-17 in AECOPD and SCOPD blank control groups were higher than those in normal control group (P<0.05), while the difference was more marked in AECOPD blank control group. The expression of p38MAPK, IL-8 and IL-17 in AECOPD and SCOPD Gln groups were significantly lower than those in AECOPD and SCOPD blank control groups, respectively (P <0.01, P <0.05). Conclusion Glutamine may exert an anti-inflammatory role in COPD patients by inhibiting p38MAPK cell signaling pathway and reducing the expression of IL-8 and IL-17.
